The way this works is by taking each sample and bidirectionally re-distributing its energy over the image plane. The result is smooth bokeh, even at low AA samples.

It is also able to do full lens simulation using a state-of-the art (polynomial optics) approach. Ever wanted to render through an 1900’s Petzval lens? Or the legendary Cooke Speed Panchro? Here ye go.
